Serving Our Community Since 1996Family Transition Support Services (FTSS) has been serving at-risk youth and their families in Mercer and surrounding Counties since 1996. We began as a grass-roots agency that focused on making a difference in the lives of children by offering them a caring, consistent role model.
Core Values
The core belief across FTSS is to be non-judgmental in our partnerships. We are a group of optimists that believe that success can be achieved by staying positive and flexible in our approach.
At FTSS, we understand that the needs and challenges in the community always change. As individual and family circumstances change, we can adapt our services to meet the challenges faced by at-risk youth and their families.
FTSS is an established Medicaid provider within the Department of Children and Families Children’s System of Care. We work collaboratively with the Division of Child Protection and Permanency, Care Management Organization, Mobile Response and Stabilization Services, Perform Care, Juvenile Justice and Family Court.
FTSS is committed to providing exceptional services and support to New Jersey’s children and families.
Our Staff
Lisa Wojtczuk Gervasi
Executive Director
Lisa earned a degree in psychology from St. Joseph’s University and completed a two (2) year intensive family therapy program at the Multicultural Family Institute. She is a well respected children’s advocate with over 20 years of experience serving at risk youth, high risk youth, and special populations.
Dr. Daisy Freer
Clinical Coordinator
Dr. Freer is our talented clinical coordinator. She has over 15 years of experience providing therapeutic support to low and high-risk youth and families. Dr. Freer is dedicated to improving the quality of life for children and adults affected by mental health related disorders.
